Patient Appliance Centre

To assist patients at discharge, Sir Charles Gairdner Hospital (SCGH) loans equipment and appliances through the Patient Appliance Centre (PAC). This equipment is for home or community use to assist with the safe transition to independence.

The equipment must be prescribed by a SCGH or Rehab in the Home Health Professional and be deemed ‘essential’ to enable you to manage safely at home or in the community.

Please bring the Equipment Loan Advice Form completed by your Health Professional when picking the equipment up. Appliance Officers require this as confirmation that a clinical assessment has been completed and to ensure that the equipment is set up appropriately.

A separate service at SCGH loans equipment to in-patients through the Hospital Equipment Store. However, if you have special equipment that you use at home, such as a wheelchair, please bring it with you if you are being admitted to hospital. Please ensure that it is clearly labelled.

 

Eligibility

You may be eligible for equipment/appliances from PAC if you:

  • have recently been discharged from SCGH (within the last 3 months);
  • are attending regular outpatient appointments at SCGH;
  • are at risk of falls, a pressure injury, or being more dependent on help without the equipment;
  • have family/ carers looking after you at home who require special equipment to do so safely.

If you are eligible for equipment under other programs such as NDIS, CAEP, a residential care facility or your care package provider you are generally NOT eligible for PAC equipment. However, we will provide the equipment for up to 3 months until your alternative funding source is able to take over hire or purchase of your equipment.

The Patient Appliance Centre is located at the entrance to the Outpatients Department in E Block, Ground Floor.

15-minute parking is available at the entrance of E Block on Hospital Avenue.


 

Equipment

Equipment includes (but is not limited to) the following items:

  • Mobility aids e.g. wheelchairs, crutches, walking sticks, frames
  • Commodes- bedside, wheeled commodes and self-propelling
  • Bathroom equipment e.g. shower chairs/ stools, bath boards, over toilet frames, raised toilet seats
  • Transfer aids e.g. hoists, slings, slide boards, bedrails
  • Kitchen stools and tray mobiles
  • Cushions
  • Temporary ramps
  • Kangaroo Pumps

 

Loan fee

Whilst there is no charge for the hire of the equipment, a fully refundable deposit is required on collection. Payment can be made via the Hospital Cashier located in the Out-patients Department (E block) next to PAC.

 

Care of equipment

Equipment on loan to you remains the property of the hospital. We ask that you look after it and return it in good condition. On return the equipment will be sterilised using an industrial washing machine.

Please contact the Appliance Officer on (08) 6457 3411 during business hours if your equipment requires maintenance that you are unable to provide or if it appears unsafe or worn out. Replacement of equipment will require further assessment by a health professional to make sure it still meets your needs.

 

Equipment returns

Please return the equipment to PAC when it is no longer required.

Collection of equipment can be organised as a service for people who do not have the resources to return the equipment. Please telephone (08) 6457 3411 to see if you are eligible for this service. Due to high demand, equipment may not be collected immediately, it would be scheduled to align with the next visit to the area where the equipment is to be collected.

PAC operates during business hours only so equipment must be picked up or dropped off prior to 4:00pm.
