GPs are central to the delivery of healthcare in Australia and at SCGOPHCG we aim to develop our services to better support GPs in caring for our community. This page provides information about our services for GPs and projects under development. We welcome any feedback and suggestions for our services.
Hospital Liaison GP (HLGP)
The HLGP supports a range of services including:
- Providing a point of contact for GP enquiries and concerns about our services. Specific queries about care that has been provided to a patient should be directed to the treating team in the first instance.
- Promoting clear and effective communication between hospitals and primary care providers, particularly in the areas of GP referral, inpatient discharge, outpatient and Emergency Department visits.
- Representing GPs as appropriate during the development of hospital and health service strategic planning, policies and processes affecting patient care and access.
- Developing a perspective of hospital and GP related issues at a state and national level.
- Working with stakeholders to provide GP education events and updates.
The Liaison GP, Dr Cory Lei, is based at SCGH for limited hours each week. He can be contacted for issues relevant to the described role above at SCGOPHCG.HLGP@health.wa.gov.au. This is for GP use only. Patients and relatives should use the Consumer Liaison Service.

Service directory for GPs (older adults)
SCGOPHCG provides a range of services to support older adults in the community and early interventions to prevent unplanned hospitalisation. This service directory provides an overview of the services that are available and the referral pathways.
Hospital in the Home (GP Referrals)
SCGOPHCG’s Hospital in the Home service is now accepting direct referrals from GPs for patients in catchment who meet criteria.
Inclusion criteria
- Patient is aged over 16 years
- Patient resides in catchment (see suburb list in PDF below)
- Patient and/or their carer consents to treatment occurring in the home
- Patient’s place of residence is assessed as safe for the provision of care (see Home Assessment Checklist in PDF below)
- One of the following diagnoses:
- Severe hypertension without focal neurological signs
- Congestive cardiac failure requiring IV frusemide
- Cellulitis requiring IV antibiotics
- Urinary tract infection requiring IV antibiotics
- Other diagnoses may be accepted at the discretion of the HITH consultant
- HITH consultant accepts governance of the patient
Exclusion criteria
- Vital signs outside normal limits (fever excepted if treatable source of infection is clear)
- Uncertain diagnosis
- Initial resuscitation in the ED required
- Continuous 24-hour assessment, treatment or observation required
To make a referral
- Call the HITH consultant to discuss the referral on 0484 714 775
- Referrals accepted 8.30am to 4.30pm Monday to Friday.
- The patient will need to attend the HITH Centre (ground floor, C block, Sir Charles Gairdner Hospital) for initial assessment and management.
- Please provide a written referral and health summary to your patient to bring with them.

Requesting medical records
Many records are now automatically uploaded to My Health Record for consenting patients including discharge summaries and pathology results.
To request information from a patient's medical record:
- Send a request to the Freedom of Information Office at SCGH.GPdesk@health.wa.gov.au or SCGH.FOI@health.wa.gov.au or fax to (08) 6457 3523.
- Please include patient's consent in your request.
- You may also call to discuss on (08) 6457 5427.
