Home Link is a multidisciplinary community focused service based at Sir Charles Gairdner Hospital which provides hospital substitution services. This service is available through Hospital in the Home (HITH) and Allied Health Rehabilitation in the Home (RITH).  The Home Link Anticoagulation Service supports early discharge from hospital or prevention of admission to hospital by managing sub-therapeutic or unstable INR levels in the community.  In addition Home Link provides an enhanced case management service for the patients through the Complex Needs Co-ordination Team (CoNeCT).

There is no age restriction for adult referral to Home Link, but patients should genuinely require services to be provided in the home. Patients who are able to travel into the hospital for Allied Health outpatient services should not be referred.

Home Link takes referrals directly from the Emergency Department, inpatient wards and outpatient clinics for patients who require Hospital in the Home, Rehabilitation in the Home or Post Acute Care Health Rehabilitation services.  Direct referrals from GPs are not accepted.

Services are usually provided for the short-medium term (2-6 weeks) and if ongoing care in the community is required Home Link staff will refer patients to other community service providers.

This program is designed to respond to the needs of complex patients who are frequent presenters to the acute hospital setting and often have extensive hospital admissions.  The program provides individually tailored service linkage, advocacy and support in the community.
