Comprehensive service to the elderly in the form of:

  • Acute medical treatment and multi-disciplinary rehabilitation for inpatients including of Physiotherapy, OT, Speech and Orthogeriatrics.
  • Consultative assessment of inpatients and community-based patients.
  • Medical consultation for ward-based, community-based, day hospital and outpatients.
  • Domiciliary consultation and community support services.
  • Residential care assessment in home, hospital and residential communities.
  • Day Hospital remedial and maintenance therapy.
  • DEED (Discharge of Elderly from Emergency Department) aimed at reducing the number of elderly readmissions through the provision of necessary community support services.
  • Day hospital specialist’s clinics in memory, continence, falls, osteoporosis, geriatric, amputees, clinical psychology, occupational therapy, Parkinson’s Group and physiotherapy.
  • Delirium & surveillance unit (DASU) – located in ward C16 SCGH specialises in the management of patients with delirium.
  • Orthogeriatric service

N.B Rehabilitation inpatients beds located in Wards 4 & 5 Osborne Park Hospital.

Patient Advice Services

Rehabilitation and Aged Care Roster Pager (DRAC Roster Pager) - carried by nursing and allied health staff members qualified to provide advice/information to patients and relatives.

Outpatient Referrals

Referrals usually received from GP via mail or fax on (08) 9346 4966.  Home visits are usually made within 5-10 working days of referral being received.

Urgent requests directed to Reception/Secretary who will page the relevant medical team.
