The GP Liaison Service provides a link between the Hospital and general practitioners working in the community. The Liaison GP, Lucy Gilkes, usually works at a system level and is only in the hospital for several hours each week. The Liaison GP position reports to executive members from SCGH and the WA Primary Health Alliance (WAPHA).
Queries from General Practitioners (GPs)
Queries from GPs regarding a patient or clinical service should in the first instance be directed to the relevant specialty Registrar. They may be contacted via hospital switchboard on (08) 6457 3333. The Liaison GP does not get involved in queries from GPs, unless a Registrar or Department is not being helpful. The Liaison GP may be contacted at email@example.com or by phone via hospital switchboard.
Queries from patients or relatives
The Liaison GP does not take queries direct from patients or relatives. These should be directed to the relevant Department or the Patient Liaison Service on 08 64572867 or SCGHPatientLiaisonService@health.wa.gov.au.
Referrals to SCGH
All routine referrals should be sent to the Central Referral Service (see below). If you think the patient should specifically be seen at SCGH (rather than another hospital) then please indicate why on the referral. If you think the patient requires immediate review (within seven days) then you should contact the Registrar of the relevant specialty – if they agree the patient needs immediate review then you can fax your referral direct to SCGH to the number the Registrar gives you (SCGH does not have a central fax). Change in GP details

Request for patient information
The GP desk is staffed 24/7. Requests for information from medical records should be faxed to (08) 6457 3523 on practice letterhead. Indicate how urgent the information is required. Routine requests are usually processed within one working day. Urgent requests can be processed within an hour or two. Requests can include discharge summaries, pathology or radiology results, or copies of outpatient letters (or handwritten Drs notes if no letter has been dictated).
How to refer to Central Referral Service (CRS)
Referrals to CRS should be made by a Dr (GP or Specialist) or nurse practitioner – do not give to the patient to send to CRS. Referrals may be sent to CRS electronically (preferred), by fax, or by mail..
